CONSENT AGREEMENT AND INJURY WAIVER

Don't let this happen to you!...Ouch!

Please consult with a physician before participating in any strenuous exercise.

In consideration of membership into Quabbin Valley Over-Thirty Baseball League (QVOTBL), you hereby agree to conduct yourself in a manner which will reflect favorably upon my teammates, fellow competitors and spectators. You understand that failure to do so may result in your suspension and/or expulsion without reimbursement of any fees you may have paid. Furthermore, you understand and agree that you are not entitled to any refund under any circumstances once the season begins.

In addition you understand that certain risks are inherent in your participation in the game of baseball and you assume those risks of your own accord and will hold harmless QVOTBL, its members, officers and Board of Directors, sponsors or field owners for any injury you may sustain in the course of traveling to and from, or while participating in any QVOTBL activity.

You have read and agreed to abide by the Guiding Principles and Rules of QVOTBL and accept all of the above conditions.

You recognize that under no circumstances will the QVOTBL guarantee your placement on any particular team. Likewise, you understand and agree that you re not entitled to any refund under any circumstances if you are drafted by a team and refuse to play for that team.

Furthermore, you certify under the penalty of long-term suspension that your birth date and age are correctly listed on the membership form.
